Endoscopic Clip Closure for Type I Perforation on the Convex Side of an Incognizable Duodenal Diverticulum During Transpapillary Lithotripsy

Abstract

A duodenal perforation, categorized as type I (duodenal perforation), type II (periampullary perforation), type III (biliopancreatic perforation), and type IV (pseudo-perforation) is a critical endoscopic retrograde cholangiopancreatography-related adverse event. The prevalence of duodenal diverticulum also increases with age. However, most diverticula grow from the concave side of the second part of the duodenum and rarely from the convex side. Herein, we report a rare type I perforation on the convex side of the duodenal diverticulum, which was a blind spot during transpapillary lithotripsy because of the duodenoscope viewing direction, and its successful management using endoscopic clip closure.
